Yoon Seo wakes up to find Shi On standing at the foot of the bed, brushing his teeth and wearing only his boxers. She screams and throws whatever she can get her hands on at him as he continues to brush his teeth, unperturbed.
While wielding the television remote like a weapon she accidentally turns on the TV, which happens to be playing a news broadcast about a man who had raped three women.
Her eyes widen as she considers what could have happened between them and she threatens Shi On, “I’m going to call the police! Stay right there!” Shi On, looking mildly confused as to why she’s causing so much ruckus merely says, “This is my house.”
She insists the house is hers as she struggles to get her cell phone from the floor. Yoon Seo falls out of the bed and when Shi On tries to help her up, she slaps him away.
Then she looks around the apartment and realizes her mistake. She asks Shi On which apartment number it is, and the embarrassment registers on her face when he tells her it’s apartment #101.
After Yoon Seo gets dressed, the two of them leave for work together. She tries to explain the situation and apologize to Shi On, but he doesn’t seem interested in hearing her out.
The truth is she had been living in apartment #101 until last week and now she lives in apartment #201. While drunk she went to her old apartment and pressed her old password, which worked. When Shi On doesn’t seem to accept her apology she tries to make small talk with him. They establish that they’re working in the same department, he’s a resident and she’s a second-year fellow. Since she’s he’s senior at work she asks if it’s okay to not use honorifics with him. He ignores her question and continues on his way to work, saying the directions aloud to himself.
It appears Yoon Seo isn’t used to being ignored and as he walks on ahead of her she wonders whether he’s stubborn or a weirdo.
In the office the other doctors wonder how Shi On is living in company housing if he’s only a resident, they all assume that his personal connections have gotten this position for him.
Do Han walks into the office and everyone acts on their best behavior. He tosses a bottle of men’s skin cologne to Yoon Seo and tells her to put it on before they visit the patients because she still reeks of alcohol. She says she can just wear perfume to which he responds that he knows she doesn’t have any. After Do Han leaves, Shi On looks at the bottle and tells Yoon Seo that he's seen that cologne in the bathhouse in his hometown. It’s the one that all the middle-aged men use.
The doctors begin their morning rounds, meanwhile Senior Nurse Jo Jung Mi is teaching the children “how not to get bullied” by throwing sand in the face of the bullies. It’s almost like he’s trying to raise a bunch of juvenile delinquents. Chief Nurse Nam Joo Yeon comes in and tells him to get back to work.
The other nurses gossip about the new resident but stop suddenly when they see the doctors coming in. Shi On is introduced to the nurses, and his (adorable) self-introduction consists of his hometown address instead of more professional details like his position or education background.
Do Han is ready to move forward with Eun Ji’s surgery, which is exactly what made Yoon Seo upset the day before. This time it’s Chief Nurse Nam Joo Yeon who speaks up against him and suggests that Eun Ji has a psychiatric examination before the surgery, but it’s no use. Do Han is determined to perform the surgery as soon as possible.
The residents and fellows follow Do Han around as he visits patients, scribbling furiously to write down his every word. Someone asks Shi On why he isn’t taking notes, to which he responds that he is and points to his head.
Yoon Seo asks him to repeat everything from their rounds so far, and Shi On rattles off all of the details he has perfectly memorized as the other doctors double check in their notes. Yoon Seo begins to scold him and tell him that writing it down is also for the patient’s benefit, but Do Han stops her from finishing.
The doctors all move along but Shi On lags behind, admiring the butterfly drawings of patient Yoo Sung Ho. The patient wakes up and begins vomiting and Shi On rushes to the nurse’s station to get the patient’s chart. The other doctors come back to see what all of the commotion is about and Shi On insists that Sung Ho needs to have surgery immediately. Do Han tells Shi On to leave the patient’s room. In the hallway the doctors argue over what treatment is necessary for Sung Ho. Do Han and Yoon Seo believe that the patient’s current symptoms are due to the fact that he is still recovery from surgery. Shi On lists all of the symptoms that point to the fact that Sung Ho needs to have surgery again and he seems to convince Do Han, at least partially. Do Han asks Nurse Jo to contact the doctor who oversaw the surgery but warns Shi On not to say or do anything.
The doctors are preparing for Eun Ji’s surgery. Meanwhile Dr. Go, the doctor who performed Sung Ho’s surgery, is playing golf and doesn’t answer nurse Jo’s call. Do Han and the other doctors look at a scan that shows that if they had waited to perform the surgery, Eun Ji’s condition would have been fatal. They begin the surgery, only Shi On is nowhere to be found.
President Lee tells Dr. Choi that she’s worried about how Chae Kyung is doing after the death of her biological mother. Dr. Choi tells her to wait it out, since Chae Kyung will come around eventually. Their conversation is interrupted when Dr. Choi receives a phone call from Shi On.
The scene cuts back to the golf game, and nurse Jo is finally able to speak to Dr. Go. The doctor can be back at the hospital in two hours, but Shi On says that by then it’ll be too late. Together Shi On and nurse Jo disobey Chief Nurse Nam and rush the patient into a surgery room right next door to where Do Han is operating.
Sung Ho’s vitals are becoming unstable, and Shi On insists that surgery must be done immediately and he’s the only one there to do it. Under the stress of the situation Shi On begins shaking and bumping into things, he’s clearly unable to perform a surgery in his current state.
Just then Do Han walks in. He quickly splits up the doctors into two teams and says that they will be performing the two surgeries simultaneously. Shi On asks to be included but Do Han throws him out of the operating room. Instead, Shi On is forced to watch from above with Dr. Choi and the Assistant Director of the hospital.
Do Han is literally performing two surgeries at once, and changes his gloves and scrubs as he goes from one room to the next. Shi On tries to offer his input via microphone but he’s silenced by Do Han.
Just as Do Han is about to return to Eun Ji, Sung Ho begins to bleed around his pancreas. The doctors can’t find the cause of the bleeding, but Shi On tells them that one of the injections given to the patient is known to cause hemorrhaging in children.
The two surgeries end successfully and Do Han is praised by both Dr. Choi and the Assistant Director. At just that moment Dr. Go arrives. He looks like he’s ready to pick a fight with Do Han, only before he can say anything Do Han punches Shi On in the face.
The other doctors have to hold Do Han back, and Yoon Seo rushes to take care of Shi On’s bleeding nose. Do Han tells Shi On that because of his reckless actions two children could have died that day. Shi On is smiling, even after being punched. Do Han, assuming Shi On is unapologetic, says he never wants to see Shi On again.
A golf ball falls out of Dr. Go’s pocket. Busted. Everyone knows that the “seminar” that kept him away from the hospital was really a golf game.
Yoon Seo sits alone with Shi On and tries to explain what exactly he did wrong. Because Shi On insisted on operating even though he wasn’t the operating surgeon who had performed Sung Ho’s previous surgery, the situation could have been dangerous. Also, focus is a must-have for a surgeon and in that operating room Shi On wasn’t able to keep his wits about him.
Yoon Seo leaves the room for a moment and Nurse Jo comes into fist bump Shi On in an act of solidarity….only Shi On thinks he’s playing rock-paper-scissors.
Do Han apologizes to Dr. Choi for his actions, and the director is understanding as to why exactly Do Han lost his cool. In attempt to reduce the misunderstanding between Shi On and Do Han, Dr. Choi explains that the reason why Shi On smiled even after being hit is evidence that he still has autistic tendencies. In instances like this his emotions aren’t accurately expressed by his facial expression, which caused him to be hit a lot by other children when he was a child.
Yoon Seo buys bread for Shi On but he refuses to eat.Do Han watches on as she tries to force feed him.
Do Han is approached by Dr. Kim, who patronizingly tells Do Han he never should have transferred departments since now pediatrics is filled with problems because of Shi On. Do Han coldly replies that any department could have problems. Dr. Kim tells him that a few days ago Yoon Seo came to his department (the Hepatopancreatobiliary department… talk about a mouthful. I’ll call it Hepatology from now on) and argued to have a child patient of theirs transferred to pediatrics. Dr. Kim says Yoon Seo must have been looking down on her classmates just because she was first in their class.
Nurse Jo is being scolded by Nurse Nam for neglecting his other patients and getting involved in the surgery. Because of his actions he will have to submit a 100 page-long apology and his pay will be cut. The other nurses listen in from outside of the door but they're forced into the room by a group of children. One child passes Nurse Jo a bag of sand and tells him to throw it in the face of his opponent so he can win the fight.
Chae Kyung is meeting with the Assistant Director. She thanks him for coming to save the hospital but mentions that she thinks it's strange that someone of his caliber would do that. He explains that while he may be in charge now, she is the one who will be responsible for the hospital in the future.
Do Han calls Yoon Seo into his office and asks her why she made a scene at the Hepatology Department. He warns her to never cross Dr. Kim again.
The self-proclaimed “beauty queen of the pediatrics department” Na Hyun comes into the hospital. She appears to be a regular as the nurses are all friendly with her. She tells them that she fainted again and after going to the ER they told her to report back to pediatrics. She sees Shi On and seems to have fallen for him at first sight.
Do Han tells Shi On to leave work at 6pm, which is unheard of for a resident. Yoon Seo follows Do Han as he leaves the room and asks him why he’s treating Shi On this way in spite of his ability. Do Han tells her that he sees Shi On as a robot with a mechanical-like medical knowledge, and although he may have some emotions, he lacks the mindset a doctor needs. In the end Shi On’s ability is just another side effect of his autism. Yoon Seo tries to defend Shi On and says that the latter just needs more time until his autism no longer negatively affects his work. Do Han warns her that if she begins to neglect her patients because of Shi On then she will also be fired.
Yoon Seo runs to catch the bus that Shi On is on. She teases him when his stomach growls and offers to make him dinner. She asks him what he likes to eat but shoots down each of his suggestions. She ends up choosing the menu herself and Shi On trudges along behind her in the grocery store.
Dr. Go is at dinner with the Assistant Director and his brother-in-law. It appears that because of what happened earlier in the day, Dr. Go is supposed to be reassigned to a different hospital but the Assistant Director says he’ll see what he can do to make sure that doesn’t happen.
Shi On and Yoon Seo walk home from the grocery store together. She asks him why he didn’t wake her up that night she fell asleep in his apartment. Was it because he wanted to watch her while she was half-naked? He replies that he was staring at her face, which looked very pretty in the moonlight. She’s pleased with his answer and walks on with a smile on her face.
Once they’re in the apartment, Yoon Seo realizes she forgot to buy oil and sends Shi On to the convenient store to get some. While he’s gone she cleans up a little, accidentally sweeping his green scalpel into the trash.
Do Han is out at a bar with Chae Kyung, who apparently is his fiancée. Chae Kyung thinks Do Han has been distant with her recently, she asks him what’s on his mind and he tells her that the hospital and his patients are on his mind. She jokes that he should cheat on her once, and maybe after a bit of “fresh air” he’d be happier. He tells her that this is how doctors usually are—they go through phases of being discontent with their lives but soon after they’re happy again.
Yoon Seo is cooking and Shi On is searching frantically for something. She asks him what he’s doing and he tells her that he’s looking for a green scalpel. When she tells him she might’ve thrown it away, he rushes outside and begins tearing through trash bags as Yoon Seo watches.
Shi On finds his scalpel, but he still looks on the verge of tears. Yoon Seo apologizes for throwing it away but Shi On tells her not to come into his house again and that from now on he’ll just eat convenient store food.
Yoon Seo’s cell phone was left behind in Shi On’s apartment,so when a call comes from Do Han it’s Shi On who answers. Do Han, not expecting a male to answer, looks at the phone to make sure he called the right number. Shi On identifies himself and brings the phone out to Yoon Seo. His message for Yoon Seo is something work related, but the look on his face after he hangs up suggests he suspects something is going on between Yoon Seo and Shi On.
While lying in bed, Shi On hold the green scalpel and remembers playing doctor with his hyung. He imagines his hyung there with him, patting his back as he falls asleep.
The next day Yoon Seo is eyeing Shi On from over her coffee cup. She asks him if he’s still mad at her and what exactly that scalpel means to him. No response. She asks if his first love gave him the scalpel and this time when he doesn’t respond she assumes it’s true.
Dr. Choi has planned a meeting with the ethics committee to punish Dr. Go. The Assistant Director suggests that they cover the entire situation up, so the Pediatrics Department as a whole won’t face scrutiny. The Assistant Director says that after going head to head with the board over hiring Shi On, they might think that firing Dr. Go is just a form of revenge. For Shi On’s sake, Dr. Choi considers covering up the situation.
In the NICU Shi On overhears as a set of parents receive devastating news from Dr. Kim—their baby is dying. Even if they did perform surgery, the child’s survival rate is still low.
Do Han comes into Dr. Choi’s office and demands to know why the committee meeting. Dr. Choi tells him that he plans on finishing things quietly with a stern warning. Do Han thinks that isn’t enough punishment for putting a patient’s life at risk, and he guesses correctly that the situation is linked with Shi On being admitted as a resident. Do Han says that he will not stand by any longer as Dr. Choi puts his career on the line for Shi On and storms out. However, when he comes face-to-face with Shi On, Do Han remembers Dr. Choi asking him to take care of Shi On.
Just then Dr. Kim comes in and grabs Do Han by the collar, reminding him that he’s already been warned once about stealing Dr. Kim’s patients.
